New games run slow in Candlekeep until I enter a building

Somebody has recently given me a copy of Tales of the Sword cost after saying that he hasn't played it in years. I had previously borrowed it from somebody else for a time, but eventually had to return it.

This time, I installed it, and got as far as werewolf island before I hit a bug that kept spawning an army of Delainys. I realized I forgot to install the patch (my game w/o TOTSC had been fully patched). I corrected this and it fixed the problem.

Now however, whenever I start a new character, the game runs extremely slow until I get inside a building. Once I change to another area, the problem seems to fix itself, even when i return to outdoors. It doesn't seem to matter where I go, just as long as I go indoors. Winthrop's inn, the building with Shank, or the barricks where you get Hull's sword all fix the problem.

Any idea what's going on? Is this normal with a patched TOTSC? I don't recall seeing it before, but it might have been a different version. It certainly didn't do this before I installed the official patch. On the other hand, it's a minor annoyance that I can live with since I get started so close to Winthrop's, and the Prolouge intro even tells you to go there and buy equipment. Not that I ever bother buying anything except some bolts...

I'm not very knowledgeable on the subject, but it sounds like the game might be reading from the CD itself at first, and reading from the cache after reentry. I've had similar sounding issues, but not with BG.
