A couple questions about Vampiric Touch.

    This is actually the first game I have used the spell. I started as a chaotic evil assassin. I can only use the spell once. The second time it always says I must wait for the duration of the first spell to run it's couse. In the spell description it says, duration=instant. So what causes the first spell to run it's course? Time?
    Second question. One of the mages from Xzar's house followed me outside and I used the spell on her. Then everyone in the area went hostile on me. Is this because I attacked her without waiting for her to go hostile first or does it have to do with vampiric touch?

    Well, did you actually go and attack someone so that you could touch them and suck the life out of them? If not, I think that's what it's waiting for. You have a certain amount of time after casting the spell to attack someone, at which point the spell will actually do something.

    And second, yes, if you attack someone if they are not hostile, that's a big no-no to everyone in the town.

    Depending on the level of the caster Vampiric Touch will cause a certain amount of damage to your enemy. So, if you are of a rather high level your foe will lose 36 or so hitpoints which will be added temporarily to yours. If you had 100 HP before casting VT, you now will have 136 for a few rounds - for duration of the spell.
    Now, when you cast it again while still having those 136 HP (read: the spell hasnt yet worn off), VT wont add another 36 HPs, only cause damage. That's what is meant with the duration message. :)
    In case the caster of VT was wounded, this spell will also heal you up to the points you drained from your enemy.

    For the second question. That happened to me, too. It also happens when Baron Ployer (from Jaheira's quest) follows me outside his house. Apparently the peasants hanging about assume you attacked someone without reason or something, as they havent witnessed what caused your opponent to become hostile.
