Problems with CD 5

    I have played my game through without too many problems but when I try to go into baldurs gate there is a problem. It asks for cd 5 so i put it in and then this message comes up:Baldurs Gate Error: media removed from drive. Please insert cd 4 into cd rom drive.
    I do this and then it again asks me for disc 5 i put disc 5 in and this screen comes up again could somebody please help me?
