    Nicademus just transported me back to the Ice Temple. I have killed everything in sight, but I can't use any of the stairways to get out. Any ideas?

    the exit stair is at east end of the map. you can only go through when you finished all the monsters. did your journal said that you finished all the monsters? (though i am not sure but i think it will update in your journal).

    2 things, if you're not patched and Nicodemos gets killed then you're stuck. Otherwise try resting, if there's one hiding this usually will bring it out.
