Alright, starting ToB, good PC, romance with Viconia active, Sarevok in party (no geas).

If my reputation goes above 18, will either of them leave my party? I was wondering if a romance was happening Viconia wouldn't leave and Sarevok won't leave no matter what because he's "bound" to you. Are either of those thoughts true?

Another thing, if you break that rep barrier, would your evil party members immediately leave? Or would it take some time giving you a chance to lower your rep back so they won't leave?

Noone leaves in ToB because of reputation. Ever. Going to Hell and back with someone (quite literally) tends to form a bond that public views cannot break. As for Sarevok, I would assume he just feels a strong connection to either you or Imoen, depending on whose soul you gave him a part of, as well as the curiosity he professes about your eventual destiny as a Bhaalspawn, and is willing to stick it out even if you go goody-two-shoes on him.

But regardless of the NPCs' motivations, Bioware saw fit to eliminate the reputation requirements for NPCs in the ToB section of the game. Even Mazzy and Keldorn, who in SoA would have not just left the party but actually tried to kill you if your rep dropped too low, don't seem to care if you suddenly start acting like Bhaal himself and slaughter everything that moves.
